RETURN MERCHANDISE AUTHORIZATION REQUEST
     
  All product returns must have a Return Merchandise Authorization (RMA) number on the exterior of the carton prior to shipping back to the factory or it will be rejected. Please describe in detail on the form below the reason you want t return the merchandise. Please know that if the returned merchandise, after inspection, is determined not to be defective, you will be charged a restocking fee.
